Release of Liability 

I hereby acknowledge and agree to the following in consideration of being permitted to participate in dance activities at The Warehouse Dance Studio. 

I understand that dance activities inherently involve certain risks, including but not limited to, physical exertion, potential injury, and unforeseen accidents. I voluntarily assume all risks associated with participation in dance classes and activities.

I release The Warehouse Dance Studio, its instructors, employees, and affiliates from any and all liability for personal injury, property damage, or wrongful death resulting from such participation. Additionally, I agree to indemnify and hold harmless The Warehouse Dance Studio, its instructors, employees, and affiliates from any claims, damages, liabilities, costs, or expenses arising out of or related to my participation in dance activities.

I have read and understood this release of liability and voluntarily agreed to it with full knowledge of its significance.

Hold Harmless

I acknowledge and accept any potential risks associated with dance activities, thereby releasing the studio from liability for injuries or accidents that may occur during participation.

Image/Video Release

Parents hereby agree to our image release policy, granting permission for the studio to utilize photographs or videos featuring their child for promotional purposes. This contract underscores our commitment to prioritizing the privacy and safety of every dancer, ensuring that their artistic journey remains respected and protected. If you do not agree to this policy please communicate that to the studio.
